Tiffany Ann Lewis, 31, of Jamestown, died Friday (April 24, 2015) in the WCA Hospital.
She was born in Olean, N.Y. on November 22, 1983 the daughter of Dawn Sloat Zofchak, Jamestown and Stephen Hibbard, Wayne, Ok.
Besides her parents, she is survive by a brother, Christopher Hibbard, Buffalo, a sister, Tamara Zofchak, Kennedy, grandparents, Walter and Ida Hibbard, Cuba, N.Y., step-father, Terry Zofchak, Jamestown, step-sister, Devan Hibbard, Wayne, Ok. and her fiancé, Tyler Neubauer, Jamestown.
A memorial service will be held at 11:00 a.m. Thursday in the Hubert Funeral Home and Cremation Services. The Rev. Mervin Lee, pastor of First Church of God, will officiate.
Friends will be received for one hour prior to the funeral service.
