Obituary of Jose M Diaz

Jose M. Diaz, 43, of Jamestown, died at 9:54 p.m. Thursday (December 15, 2016) at his residence.
He was born in Patterson, N.J. on August 3, 1973 the son of Nicolas Diaz and Marta Martinez of Puerto Rico.
He was employed at the Connection in Jamestown.
Jose is survived by his fiancée, Misty lohman, three children, Jose Jr. and Misty Diaz, Jamestown and Jonathan Diaz, Omaha, Nebraska, five siblings, Jeannette Diaz, Winston Salem, N.C., Nicolas Diaz, Puerto Rico, Nestor Diaz, Patterson, N.J., Emmanuel and Joshua Diaz, Connecticut.
A funeral service will be held on Monday at 11:00 a.m. in New Creation Assembly of God Church. The Rev. Juan Perez, will officiate.
Burial will be in Lake View Cemetery.
The family will be present to receive friends on Monday from 9 to 11 am in the church.
